To apply for the Pro Player Spring Tournament:
1. Fill in the Application Form on the computer and print out a copy, OR print out a copy and fill it out with a pen.

2. CLICK HERE for the Pro Player Camps MEDICAL RELEASE FORM. This must be completely filled out and brought to the first day of camp.

For consideration of being allowed participation in the Pro Player Lacrosse Tournament and related activities, the undersigned:

1. Acknowledges and fully understands that each participant will be engaging in activities that involve risk of serious injury, including permanent disability and death, severe social and economic losses of which might result not only from their own actions, but from the actions of negligence of others, the rules of play, or of any equipment used, further, that there may be other risks not known to us or not reasonably foreseeable at this time.
2. Assume all the foregoing risks and accepts personal responsibility for the damages following such injury, permanent disability or death.
3. Release, waive, discharge and covenant not to sue Pro Player Lacrosse Camps directors, officers, officials, emplyees, other participants, sponsoring agencies, owners and leaseholders of premises used to conduct the event of which are herein after referred to as “releasees”or otherwise.
4. Further, I agree to allow my child to be treated by the Athletic Trainer in the event of an emergency or injury.
**I for myself and on behalf of my heirs, personal representitives and next of kin have read this release of liability and assumption of risk agreement, fully understand its terms, understand that I have given up substantial rights by signing it, and sign it freely and voluntarily without inducement.
